How long does it take?
About 10 minutes. Everything is online, nothing to print, and you can do it from your phone.
Can I just fill them out at the office instead?
Of course. Doing them ahead simply means your visit starts on time instead of at the front desk. If you come in early, plan for about 10 to 15 extra minutes.
Who fills out a child's forms?
A parent or legal guardian completes the Children's forms before the visit. If you have more than one child coming in, complete one set per child.
What should I bring to the first visit?
A photo ID, your dental insurance card if you have one, a list of any medications you take, and recent X-rays from a previous dentist if you have them.
